You need to continue thyroxin and keep monitoring tsh. For diabetes start lifestyle management, diet control (avoid fried, sweets, bakery, juices) and start exercise. If still uncontrolled then consult again.

You may require to increase the dose to 75 mcg per day on empty stomach, but do not start it yourself please consult you doctor in this regards.
