Asked for male, 35 years old from Delhi

I am 25 week running now. Have history of cerclage at 16 week. On 9 jan doc did scan in it cervical length was 2.59. In yesterday's scan she said it's 3.does the length changes so abruptly? And since today morning I noticed some reddish brown discharge just before periods and mild pain in lower abdomen and back pain. Baby is kicking but discharge is my concern. I called my ob she said may be due to cervical irritation and added trenaxa tab. And stop heparin and aspirin. Shall I go to another ob for scan m worried am I going in premature labour pls help?

There are inter observers error in measuring cervical length. Switching over your ob is your preference. if there is no pain and no spotting , nothing to worry. these things are due to usage of heparin and aspirin and can be temporarily be with held

I am patient of rheumatoid arthritis. I consult doctor and start some of medicine 6 month. But my esr (36 to 27) and some other level are still up and down. So doctor tell me if your tb report is normal than we started biological medicine. But my tb gold lithium heparin report come 1.41 and doctor says before we started biological medicine for arthritis first we solve latent tb. But my lungs are completely normal and physician says I don't need any kind of tb therapy. I also do mantoux test result is 2 and normal. My question is can I star therapy of biological medicine in arthritis or constant other tb specialist.

Urs tb gold report is good and tb gold is not a gold standerd test for tb.
If lung clear thn you can start ra treatment .but for confermation you can go for hrct of chest and sputum test.
For private counsltation send urs report of hrct and sputum test.

Hi, i'm 28 years old. I was suffering from morning n bed time back pain. I consulted rheumatologist, he suggested that my muscle strength was weaken because of daily heparin in 1st pregnancy. So, he suggested me to take calcium rich foods n pills. I was using supracal hd, n food also. But one of my grandma's suggested have barley daily for pain. As of her suggestion, I was having daily, so pain is reduced. But, I have a doubt that is it ok to have barley on daily basis. Is it effects any my next pregnancy. Please suggest me.

Asked for male, 70 years old from Hyderabad

My father 54y/m, hypertensive (well-controlled) and no other significant medical history underwent partial glossectomy and unilateral neck dissection for stage 1 carcinoma of tongue on 2/4/16. He was quite stable for 4 days post-operatively. On 5th post-op day (6/4/16, he had sudden cardiac arrest and is in unconscious state since 11 days. He is admitted in icu under partial ventilator support since then. All the investigations done including mri and ct of chest, brain along with cardiac assessment was normal (no significant findings. He is currently diagnosed with post cardiac arrest hypoxic encephalopathy and is only on supportive management including anti-epileptic drugs and heparin. There is no significant neurological recovery since 11 days and currently having myoclonic jerks and focal seizures frequently. I request you to kindly inform the required management for the above explained condition.

I am pregnant 8 weeks. Previously, I had an abortion in June 2016 (bleeding/no growth of foods find in ultrasound). Now, At this time, following tests is conducted by Doctor (Gyeno). Antiphospholipid antibody IGG- 38.18 GPL U/ml IGM- 16.66 MPL U/ml Anti cardiolipin IGG - 41.13 GPL U/ml IGM - 18.01 MPL U/ml Above results are out of range. Prothrombin Time -12 second International Normalized Ratio- 1 Above results are in normal range. I want to know that Based on above test results, is there I need blood thinning treatment during pregnancy because Dr. Giving Following treatment to me:- -Ecosprin 75 mg (daily) - Clexane injection, low molecular weight heparin (0.4 ml/40 mg)(daily), both for 8 months. Also, please tell me if bones (me & my baby) becomes weak with blood thinning medicines?

Hello first pregnancy- 5th month amniotic fluid reduced. 6th month high bp took sachets and hermin iv. No improvement iugr occurred. 7th month induced abortion. Baby weight-600gram second pregnancy- ecosprin 75 before pregnancy. Heparin injection started during 2nd month only. 4th month amniotic fluid reduced and iugr started. 5th month took hermin on alternate days. No improvement. Induced abortion during 6th month. This time no bp. Babies weight 300 gram only we done fetal autopsy ie normal also we done baby dna sample test in genetics dept. Ie also normal. We did starch test after 1 year from abortion toxoplasma igg -7.53 rubella- igg-51.70 cytomegalovirus igg - 158 herpex simple virusigg -48.30 these show high compare to normal now we have to plan for 3rd pregnancy what are the solution for this thank you.

Hi. Kindly do apla, protein s and protein c deficiency, ana test to rule out pathology.
As soon as periods date skips, confirm your pregnancy with a scan and start on high dose aspirin and heparin once there is intrauterine gestational sac. Closely follow up with fetal doppler check for uterine anomalies like arcuate uterus or septate uterus prior to conception. Plan for prenatal vitamins for both husband and wife 3 months before planning to concive

My wife's age is 35. She has sle but in remission. She had a miscarriage in july 2019 at 7 weeks. (before that also, 2 miscarriages happened but that time she was taking methotrexate so don't want to really count those). She was put on ecosprin, heparin, duphaston from 6 week onward in the 3rd pregnancy. She takes, 2 gm saaz ds daily, 300 mg hcqs. Currently all the tests like, ana profile, dsdna, ro/la, c3/c4, tsh, apla (negative) and many other antibodies all show negative right now. And Dr. says that disease is controlled right now and you can plan for pregnancy with having ecosprin 4 weeks prior to conception and reducing saaz ds to 1.5 and hcqs to 400 mg. Kindly suggest what should be done. Either try once again or think of surrogacy as don't want to go for one more dnc. Please reply.

Yes from the report it seems that the sle is in remission phase. Don't worry, sle in remission is the best time to conceive.
Have seen several successful outcome in sle after few initial losses. Go for the pregnancy, once you conceive apart from ecosprin, hydrochloroquine (hcqs), progesterone support, also use lmwh empirically. Need to have a regular antenatal check up. My liver ggt levels are high since last 6-7 months. I have stopped alcohol completely since then. I used to drink on weekends. The level is 114 iu/l.other levels are in range. I am not getting hungry since then as well. Total loss of appetite. I tried livomyn syrup, aptivate syrup, aristozyme syrup, betonin syrup etc. I consulted a gastroenterologist his medication also didnât work. The doctor gave these tablets: alcomax,samford, udiliv,highday tablets and bevon capsules. I also had these tests done: lft,usg abdomen, immunoglobulin igg, ceruloplasmin,soluble liver antigen (sla, liver-kidney microsomal lkm 1 antibody, smooth muscle antibody, anti nuclear antibody ana, transferrin saturation, hepatitis c tests hcv, heparin b surface antigen hbs-ag both were non reactive etc in all these above tests only immunoglobulin igg was 16.86 in which normal range is 5.52-16.31.others were in range & negative. The doctor said everything is fine and gave the above medication. But ggt yesterdayâs report is high.

My wife: in 2014 left ovary was removed due to huge cyst and also dermoid, After one year right ovary torsioned. Doctor saved 40%. 3rd ivf - icsi cycle Gonal rFSH 450, with menopure. 5 egg retrieved (22 Jan. 5 fert. 2 day 3 transfer (25th jan, 1 day 5 transfer (27th Jan) and 2 freeze. Beta HCG after 18 days of egg retrieval (9th Feb: 1300 Beta HCG after 25 days of egg retrieval (16th Feb: 69260 Currently on progesterone supplement 3 times a day. Progynova 3 time a day. Heparin inj once Daily. HCG 5000 iu sc. Every 5th day after transfer At 6 week scan all three embryos now attached with confirmed heartbeat. Doctor has advised for embryo reduction. My question is whether to go with two or one baby. Which is more safe with respect to my wife's health? Whether to go with twins or singleton? Her age is 30 and weighing 45 kg with height 5 feet 3 inches.

You should go for reduction of one embtyo.
Twins can be caried till 34-36 weeks if you follow your doctor's advice ,take adequate rest and your medicines regularly.
