There are two things I suspect could be cause of your severe joint pain. One is rheumatological and another is neurological. Although I think the former is more likely that later. It could be Rheumatological problem (rheumatoid arthritis, osteoarthritis etc) because both your hand joints are affected and you are also experiencing stiffness of these joints. You have been longstanding diabetic, as per your own words not well controlled, this could be a complication of diabetes rather than the medications that you are taking.
