You should probably have a scan of your kidneys and consult a nephrologist to make sure that there are no renal causes apart from diabetes. Telmisartan can transiently change creatinine levels but it protects the kidneys. How is your blood pressure. Try to consult a doctor.

Your blood pressure is quite high despite taking telpil (40) od. You should at least get some investigations to rule out secondary complications of hypertension in form of cbc, rft, rbs, lipid profile, ecg, echo, fundus examination. Usually if BP is well controlled with a single drug, then there is no need for additional drugs. But in your case doctor has already recommended Telmisartan, Chlotiazide and a beta blocker, means there is need for these drugs. Since no readings are mentioned, it is difficult to comment. Thanks.
