Hello asabdullah, since telma is a long acting drug, od dose (once a day) is preferred rather than twice a day. So 80 mg once is better than 40 mg twice. Usually first 40 mg once a day is started (provided renal function tests are normal) and if bp not controlled, then can escalate the dose to 80 mg once a day. Also concentrate on diet and lifestyle modifications.

140 not too bad. will she accept telma 80? It is not as strong as telma CT, but is more effective than 40.
or you could wait and watch till it rises to over 145
