Hi, from the information you provided it appears that you have major depressive episode. Though duvanta is an otherwise good drug, you have tolerability issue with this drug. You should consult psychiatrist to replace it with other, more tolerable alternatives like ssri or mirtazapine. You must take some anti-depressant so consult asap. Best of luck.

There could be other causes of your symptoms. It could even be a blood supply issue. You will need to be re evaluated for the symptoms. Curtailing smoking /tobacco/ alcohol if any will help.

Good to know that your depression and OCD symptoms have reduced to a point that you no longer need medication. Though you doctor slowest reduced your dosage but it seems your body is showing withdrawal symptoms. Plan one day at a time and start doing small things. Slowly your body will gain strength and you will be able to do more. Don't stress too much. If you don't see any improvement even after 15 -20 days, consult your doctor.
