There could be a lot of reasons for her pain, it may not actually be linked with her previous history of breast cancer and could just be degenerative arthrosis setting in the joints. I will advise for a fresh x ray (ap standing & lateral views) of affected knee and get her esr, crp, serum electrolytes and serum vitamin d levels done so that I can see and advise accordingly. At this stage there can not be any advise except observation and intercourse around ovulation.
Serial ultra is better indication for ovulation.

Not able to conceive to conceive for 2 years along with endometriosis warrants tubal testing or hsg. If fallopian tubes are patent, try for iui. No use doing the same treatment again and again. Iui is also equal to natural conception as the fertilization is natural.

Pcod has to be corrected. Weight reduction should be done. Physical .activities can be increased. Then only induction drugs will work.

It all depends on your parameters. If you have PCOS, letrozole is better. in other cases, clomiphene is better. Each has it's benefits and risks.

Letrozole is used for ovulation induction means it helps the egg to grow. If you have egg growth this month without letrozole, then egg will get released and you can expect pregnancy if you have had sex during egg release time/ovulation.
In case egg growth has not happened like during your previous cycles due to pcod then your cycles may be delayed.
However it is better to check for pregnancy if your periods are delayed and if you are expecting pregnancy.
