Asked for male, 36 years old from Lucknow

This 33 years old female presented with c/o fever 1 week, breathless weakness, loss of appetite 1 week, bp 110/70mmhg, p/r 1116/min, temp normal, cvs s1+s2 normal, chest right low air entry, cns cons & oriented. Patient investigated & diagnosed as above managed conservatively by iv antibiotics (cefobran, ppi and other symptomatic treatment. Pleural tapping done on 25/02/2020 under la with aseptic precautions from pleural cavity & 1500 ml straw fluid aspirated. Post procedure uneventful. My att started on 29th feb 2020. I am taking same dose since 29th feb 2020. Kindly guide me about dosage whether I have to reduce or take same dosage. Medicine details: rifampicin isoniazid pyrazinamide and ethambutol hydrochloride tablets ip forecox each film coated tablet contains: rifampicin ip 225 mg isoniazid ip 150 mg pyrazinamide ip 750 mg ethambutol hydrochloride ip 400 mg dosage: 2 tablets per day omez d omeprazole pyridoxine hydrochloride tablets ip 40 mg benadon.

ATT is usually taken for 6 months. After 2 months, pyrazinamide is stopped and other medications should be continued.
In your case case, i suggest you to repeat a CXR to look for the response and amount of remaining fluid. Also continue rifampicin, isoniazid and ethambutol for atleast 3 more months. You can stop pyrazimamide.

Im am 25 years old, present weight 55 kgs (70 1 years before, I am suffering from stomach pain from 1 year in frequent intervals. Im not gaining weight and always burping is the main symptom. Usg and ct scan shown some problem at lower abdomen ileal area ,linked pelvis and appendix, recent usg shown mass formation in the right side of pelvis and enlarged right iliac nodes. My doctor said it is intestinal tb and started treatment. My doubt is how he confirmed as intestinal tb? And he said I have have to take tablets for 1 year and given combunex800 (ethambutol hydrochloride 800 mg and isoniazid 300 mg) and some becoplex tabs, allergy tabs. But I came to know tb treatment will 6 months with 2 months 4 tabs and next 4 months 3 tabs. Why my doctor said treatment will be 1 year and and gave 2 tablets only?

June 2018 - started dialysis as kidneys failed oct 2018 - diagnosed with tb (was taking r-cinex 300/450, pyzina 750 combutol 400, benadon 40 jan 2019 (i was on dialysis & tb medication) I started losing vision developing numbness in my feet which increased till thighs (almost both legs completely). April 2019 - Dr. terminated tb medicines as I had really bad vision due to isoniazid may 2019 - vision/eyes almost recovered aug 2020 - I consulted neurologist, he did not perform any outside tests, used the instruments & pins he had, advised rejunex od capsule once daily. He wrote that I had small fiber neuropathy, features of peripheral neuropathy mar 2021 - there is still numbness in my both legs, I never had problem in walking. Though I am still taking rejunex od. Please advise what to do.

Dear lybrate-user,
your doctor has diagnosed your problem as a peripheral neuropathy and investigations are not going to add anything extra, the detailed examination by your doctor using "instruments and pins" is commendable. He has treated you also corectly. Kidney disease is often complicated by peripheral neuropathy. I sincerely advise that you follow your neurologist.

Health Query

Can someone tell me what is the treatment schedule for tuberculosis in india as per who, please don't give me diplomatic answer, like talk to nearest tb hospital or pulmonologist and all. As I had it all .no body is clear regarding this treatment every doctor telling same thing. If one have been diagnosed with tb (test results- sputum for afb negative, hb-10, esr -99, rest all cbc normal, tb gold test - positive) physical symptoms cough lasts for 20 days, fever for 5 days no weight loss, no continues fever, doctor asked to start tb treatment it's been 4 months on treatment, but my question is should we continue with all the drugs as we are on (rifampicin 450, isoniazid 300, pyrazinamide 750, and ethambutol 600,) for six month or there is any change in treatment schedule. please clarify.

In fresh tb case rehz for first 2 months and then maintenance dose reh for n3xt 4 months in most cases if response to drugs good. Pt need review every 2 months.

My son is 3 and a half years old diagnosed with spinal tb. He had recurring fever and localized lower back pain. There was pain in his legs behind the knees and stiffness. He had difficulty walking. His gait was antalgic. His mri showed l5 s1 affected with epidural abscess. Mis bilateral decompression was done. They started him on att 3fdc (pyrazinamide 450 mg, rifampicin 225 mg, isoniazid 150 mg based on his weight 15.6 kg) and wysolone 25 mg (steroid prescribed for 1 month and gradually tapered and stopped at the end of 2 and a half months). After 3 months there was a recurrence in tb which spread to l4 and l3. To make the spine stable we have done instrumentation. Is this recurrence due to wysolone?

Att to be given according to your weight. Drugs are correct just check for dose. And do continue as advised and complete the course. Do consult your pulmonologist timely.

I am going through the Potts spine treatment. The doctor prescribed level one medicines for the treatment. However after a month and complete blood tests the liver test showed high SGOT AND SGPT. So the doctors have stopped Rifampicin. Pyrazinamide. Both of these medicines and says will resume it once the liver levels get normal! I am worried as I am bed ridden for almost a month now.

My father aged 68, some 8 months back was diagnosed for Spine TB and he started on AKT4 for 2 months, AKT3 for the next month but had to stop due to vision loss and following 5 months was taking Rifampicin and Isoniazid. His vision is almost back with some eye power complaint. Recent blood report shows hemoglobin at 11.5. He has recently started to complaint about burning sensation while peeing and frequent urination. His Blood urea, BUN is high and so is creatinine = 1.77. I read that Rifampicin could induce such problems. I consulted general medicine doctor and he was concerned and asked to remove azulix (sugar medicine) and include nefrolex for 5 days and then redo the KFT. however he thing rifampicin could rarely cause this kind of toxicity. FYI. He is taking 40 mg of pyridoxine, his blood work shows platelet count=130, ESR 70, CRP has come down from 67 to 8.4, LFT results are normal, His back pain is almost gone. Remarks of Hemogram: RBCs: Mild anisopoikilocytosis. Predominantly macrocytic normochromic with macroovalocytes. WBCs: Mild Leukopenia is present. Platelets: Appear mildly reduced in smear. Macroplatelets are seen. Urine has pus cells loaded.

My age is 28, height is 5'9" & weight is 85 kgs. Mujhe backache rhta hai. Tests me spinal TB show hua. Mera treatment 3rd aug se start hua. Jisme maine Rifampicin (R)=450 mg/day & Isoniazid (I)=300 mg/day & Pyrazinamide (P)=1500 mg/day & Ethambutol hydrochloride (E)= 800 mg/day liya. After consulting another doctor on 17th aug 2018 maine dose increase krdi. So from 18th aug 2018, I am taking R=600 mg & I=300 mg & P=2000 mg & E=1200 mg. But still my pain has not gone. I want to you ask about this. Mjhe back pain me improvement bhut kam h.kaafi pain h abhi bhi. Mjhe painkillers lene pdte hn regularly. Ye pain kab tak jayegi? Ya aur kuch h jo mujhe improve karna chahiye. Any tests or something jo mujhe krwane chahiye?
