As per standard protocols, there needs to at least 5 years of seizure free period before tapering and stopping medications. Newer protocols followed by some doctors believe in stopping medications even if there is 1 year of seizure free interval. It is best that you consult with your neurologist and let him decide on what is best for you.

Your sugar levels are very well controlled. Please make sure you don't get hypoglycemic episodes otherwise dose adjustment has to be done.

Pain in rectum or anus has nothing to do with pain in penis. For anal pain I would suggest to consult a surgeon to rule any fissure. I am not sure why you have pain in penis and more so when you have pain. During limp state or erection? you may try simple pain killers if it may help.
