I am 37 years old men from gujarat. I did m.pharm and was a lecturer in pharmacy college at Gujarat. When I was talking my lecture on 10/03/2010 suddenly I felt fear n hallucinating condition and I diagnosed at that time with a paranoid schizophrenia n I was on combinations of medicine like antipsychotic n all but after that my doctor has told your disorder has been concerted to bipolar disorder n right now I am on lexpin 25 mg, lithium bicarbonate 450 mg, divalproex sodium 500 mg, nortriptyline 50 mg, trazodone 100 mg, fluoxetine 20 mg, nitrazepam 5 mg, trihexyphenidyl 2 mg on daily basis at night n this all medicines I am taking regularly although I am getting 2 to 3 episodes per year. Right now I had tried brintellix 10 mg for a month but I am getting depressed so I had stopped right now brintellix. Actually I am feeling tired from this condition n getting suicidal thoughts, lack of confidence, worthlessness n getting angry easily n feeling alone. Really I am feeling helplessness. Please can anyone tell me whether my treatment going is on a right way or not. Also I had taken 4.4.2 ect. And right now I am thinking about ultra brief pulse therapy n rtms but getting confused about this because this requires more sessions so financially also it will goes more. But ya if it gives a benefits to my conditions then I am ready to go ahead. I had taken many advice from various doctors in gujarat. Today I approximately passed 10 years with this illness. So please can anyone suggest me a right way. Thanks.

I think a complete course of ects with medication will make you feel better. If affordability is an issue am sure there are good medical colleges nearby where a psychiatry department can treat you at affordable cost of treatment.

Dear Doctor, Psychiatrist I am having problem with occasional anxiety and fear of going far away from home (also fear of going in bus, train, plane) and get anxious in traffic, traffic signal and standing in line for shopping or waiting for purchasing ticket in line) having fear of getting panic/anxiety attack. Earlier, I had 9 month course of pristiq 50 mg ER (which helped), then psychiatrist replaced pristiq with (1 mg clonazepam, 0.5 mg alprazolam in the morning and 10 mg nitrazepam and 1 mg clonazepam in the night) I slowly tapered off from the 4th month itself from half of it to and came out of it in 10 month (very very low dose 0.25 mg clonazepam and 2.5 mg nitrazepam), (I had similar effect both from starting dose to ending dose) when I stopped, again I felt anxious, and different psychiatrist put me on 20 mg paxil (helped with anxiety but problem was always feeling drowsy) came out of it in 9 months. Then started taking some natural supplements and vitamins, since 8 months, now in 8 months I had anxious feeling 3 or 4 times only when I didn't get proper sleep or caught in traffic delay). Now my main problem is going far away from home (also fear of going in bus, train, plane) and get anxious in traffic, traffic signal and standing in line for shopping or waiting for purchasing ticket in line) having fear of getting panic/anxiety attack. My question is, 1) should I continue with natural routine to help completely coming out of agoraphobia? Is it a possibility? 2) should a course of SSRI/SNRI, (say for 9 months and assume) which will make me normal again and free of agoraphobia, and then discontinue the SSRI/SNRI. Is it possible that, I can be free of anxiety and agoraphobia without SSRI/SNRI after stopping SSRI/SNRI? (please note that, earlier SSRI/SNRI had side effect of sleeplessness and low sex drive, so I was not interested in it, but if it helps in long run say after 9 months or 1 year, I can be completely free of anxiety and agoraphobia, the I can go through the side effect? 3) If I do a say, 3 or 6 or 9 month course of alprazolam and clonazepam, and assume, which will make me normal again and free of agoraphobia, and then discontinue the alprazolam and clonazepam. Is it possible that, I can be free of anxiety and agoraphobia without alprazolam and clonazepam after stopping alprazolam and clonazepam? Please note that my anxiety/panic feeling is not very severe, but thinking of discomfort (in last 4 years, I had total of 4 or 5 panic/anxiety attack, lasted in less than 10 minutes, mostly shortness of breath and sometime heart palpitation or chest tightness) when going far away from my home makes me avoid the situation. I really appreciate your detailed guidance for my above query. Thank you very much for your help.

Apart from taking symptomatic treatment I will suggest you to take injection vitcofol 2 cc intramuscularly every alternate days for five pricks over the buttocks through a health professional and then we can think of tapering the dosage through your psychiatrist.

Dear sir, my age is 36 years & I have been married 8 years & 7 months ago, I have two daughter & they are 7 years & 3 years old. Sir, before marriage I was involved in many wrong activities, many bad habits such as eating gutkha, tobacco,heavy smoking (chain smoking) heavy addicted of alcohol, charts,gaanja & also the chemical drugs like nitrosun etc. Most of these habits were left by me many years ago from now. All of the above habits had been left by me in the past 8 to 11 years one by one & all these habits were adopted by me in very early stages of my life i.e. From 15-16 years of age. 10 important years of my life I have lost in these addictions ,now every habit I had left by my own confidence. Only tobacco chewing is with me now & that to is in very low quantity. Also I have been using sex workers for my sexual requirements (safe sex) from the age of 16 & it just stopped after marriage. The usage of sex workers does varies from 2 to 4 times per month. Sir here mention above are the things which I have done knowingly or unknowingly for the past many years before marriage. Now I have been married and I have left all the things. My married life is going satisfactory but I think that I am suffering from some sexual disorders & that I am briefing you. My wife haven`t told & she also does`t know about this ,but it is my own consideration that I am short falling. I don't know whether these disorders are due to my past activities or due to work stress or due to any other reason. I think I am physically fit having height of 5 ft 6 inch & weight of 74 kg. I used to do exercises for keeping myself fit before marriage. I have done sex before marriage both with sex workers & my girlfriends but at that time I haven't considered that I am short falling in the intercourse with them, but now after marriage, I realized that my intercourse time is quite low, I think that I am suffering from premature ejaculation or erectile dysfunction or whatever it is. For my problem, I have previously consulted with one ayurveda doctor online, but after getting treatment from him for about 7-8 months, I found that I performed better when I was taking medicines. Then after stopping the medicines I found that I am in the same state where I was before starting medicines. Also my financial conditions made me to quite that particular medication as it was to high. Please suggest me the medication & doctors for the same case.

Hi your father has been eating nitrazepam (nipam) which is a benzodiazepine for several years and just became tolerant to it that's why he is having sleep difficulties.
He should not consume alcohol which is also a depressant like nipam and is really dangerous for his health cause both of them act on gaba pathway in brain.
