YOu are on a good combination pill ad if there is no reason to change it you can continue it. Trilosar 6.25 mg Tablet is a combination of three medicines: Losartan, Amlodipine and Chlorthalidone. Losartan is an angiotensin receptor blocker (ARB) and Amlodipine is a calcium channel blocker (CCB). They work by widening and relaxing the blood vessels, thereby lowering blood pressure. You can take osar 25 twice a day and levothyroxine early morning before any food is taken and do not take milk with it. Gabapentin can be taken anytime and pantoprazole half an hour before breakfast.

If diastolic is always below 80 and systolic rises to 150 on most of the evenings then recheck after 15 minutes of rest. If still above 140 then add Tab. Amlong 2.5 in the evening around 6 pm.
As for your second question. All three drugs are almost same on action potency and lil different in side effects. So doesn't matter which one you take until your kidneys are fine.

She requires change a slight change in medications, a combination of medications, hopefully she may have already had echo, ecg, kidney function tests etc to find any causes that may be leading to this hypertension. Also please ensure she is taking lifestyle measures like salt control in diet, daily 30 mins of walking and eating a healthy diet (you can take detailed diet precautions for the same)
