Ideally gap should be at least 4--8 weeks between two doses of same vaccine. Duration between two vaccines even works even if doses are forgotten.

5 years, but if you got infected after 3 months of vaccination, you have to take two vaccine, on day 0 and 3.

Please wait for another 5 days. The baby should be 45 days old. Diarrhoea (Rotateq/Rotarix and pneumonia vaccines are optional vaccines but it is better to give them.

There are a few brands available in the market. The age for the vacination is 9 to 26 years of age. A family doctor who gives the vaccines can prescribe it probably.
