you should take your blood thinners and other medicines regularly and on time. With a BMI of 37.89 kgs/M2 you close grade 3 obesity (morbid obesity). This is the main reason for your high BP and the need for various medications.
Doing half an hour walk and 15 minutes of yoga (?) is not at all enough. You will have to seriously look at reducing at least 5 to 8% of your current weight. With weight reduction your BP will also come down towards the expected level of 130/80 mmHg
