Treatment of PTB is usauly by 4 drugs and dose is calculated on weight basis.As you are diabetic,your intensive phage should have been for 3 months and not for 1 month and also maintenance phage of ATT should be of 6 month,so total of 9 month treatment has to be taken. You should have periodic xray chest, sputum AFB and liver function test done.Your diabetic should be under control ,otherwise all treatment will be waste.

If size of lymph nodes have decreased or stationary with no TB toxaemia symtoms then continue TB treatment to complete minimum of 6 months treatment.

You can stop all the medicines and get a lymph node biopsy. If positive restart akt and if negative then no need of treatment. Sometimes lymph node don't reduce in size beyond a certain size in tb lymphadenitis and they persist.

There is no reason for you to continue Pyrazinamide so long. And why is there no isoniazid in the prescribed drugs. The regimen you using for to cure tb is not correct. It should contain rifampicin, isoniazid, Pyrazinamide and ethambutol for 2-3 months and later the drugs should be continued without Pyrazinamide only for 9 months at the most in total.
I don't understand the reason for 1.5 yrs of treatment. Even if it's bone tb the drugs should be for 1 year and without Pyrazinamide.

your doctor has diagnosed your problem as a peripheral neuropathy and investigations are not going to add anything extra, the detailed examination by your doctor using "instruments and pins" is commendable. He has treated you also corectly. Kidney disease is often complicated by peripheral neuropathy. I sincerely advise that you follow your neurologist.

Rule out drug induced hepatitis by getting lft done. You was on wrong advise as ciplox need not be added for first 4 month. Pza in akt4 need to be taken only for 2 months. You need addition of ethambutol to rcinex in continuation phase.
