Dear lybrate-user did you had fever or prior to the injections. Pancytopenia is a blood picture which can be obtained in multiple conditions since I have no access to your blood reports it will be difficult to tell the cause for your pancytopenia.
Ferric carboxymaltose can sometimes cause fever but the fever should settle in 24 hours if it lasts more than 24 hours you may be having a coincidental infection. Meet a physician and get yourself evaluated for persistent fever.

Hello Mr. lybrate-user. I understand that your wife is suffering from Iron deficiency anemia. However, it will be prudent to see the reports before knowing why exactly she is not improving with oral medicines. You need to consult a Hematologist. Do get the Following tests done: 1. Serum Ferritin
2. Serum Iron, TIBC
3. Direct Coombs Test, Reticulocyte Count
4. Hemoglobin HPLC (test for knowing Thalassemia status)
