Hello sir,
What are hemorrhoids? — Hemorrhoids are swollen veins in the rectum. They can cause itching and pain. Sometimes they can also make you bleed during a bowel movement.
In some cases, you can see or feel hemorrhoids around the outside of the rectum. In other cases, you cannot see them because they are hidden inside the rectum (figure 1).

I am 21 years old and living in the United States and from September 2016, I've been suffering from rectal bleeding and pain. It's recurring, sometimes stays for 3-4 weeks or even more, sometimes for 2-3 days. II've started eating fibrous food so my stool isn't even hard anymore, but still the rectal bleeding keeps back coming after every other week. The doctors here gave an external rectal exam and found no external hemorrhoids, and suggested that either they're internal or it could be a fissure. They gave an ointment, Hydrocortisone Acetate Pramoxine HCI cream, which didn't work for me. When I pass stool during these times, it pains a lot, almost unbearable. And on the tissue paper, there's bright red blood. I take warm showers and eat fibrous food, but the pain remains for at least 12-14 hours.

You might be suffering from a fissure in ano.
This may not be an acute fissure, as the doctors were able to do per rectal examination, suggests you don't have anal spasm.
I suggest you to start SITZ BATH for 10-15 minutes for 3-4 times a day.
To reduce the anal sphincter tone NIFEDIPINE ointment is usually prescribed with LIGNOCAINE.
I suggest you to get a prescription for the same.

I am suffering from chronic fissure, with little skin tag outside anus Sentinel pile. Initially I was having harsh pain in anus, so I went to doctor for treatment. He told there is crack in fissure. He has given me medicines for 2 weeks. Medicines are as 1.puffbuta,2.severe3.wellorab, 4.softovac - bowel regulator. One week past, today I again went to doctor to ask regarding skin tag, asked whether surgery is required. I also told there is much relief in my anus pain. But I am scared about Sentinel pile. Doctor said there is no need of surgery, there are many cases like this which are treated and cured by medicines without surgery. Also doctor said why you want to do surgery as it will have a harsh pain. And you will have to sit in home for 2-3 days after surgery. I am very much scared with surgery. But I am very much scared with skin tag. The doctor had also given me smuth cream which contains hydrocortisone acetate, calcium dobesilate, zinc. This is to be inserted deep inside anus. Currently I am not using it, shall I use it even if I have skin tag. I also don't know whether this fissure will be cured by medicines. I am very much scared. I have also started drinking milk 1cupwith lemon crushed in it, for better results. Also I am taking sits bath as told by doctor one to two times in a day. Please help, (Please briefly explain my problem my query that whether I will get rid from skin tag without surgery, and also suggest if any other remedy to get cure.

I have evaluated your query thoroughly. Fissure gets better with medicines, sitz bath and keeping stools soft.
Fissure can happen again any type in case of excessively hard stools.
Even after operation fissure can happen again.
There is no cure of skin tag without surgery
But we do not recommend any type of surgery for skin tag, because it does not create you problems
