Pramipex is a medicine not usually prescribed by Orthopaedic surgeons so i cannot make a comment on that.
I would want you to rule out :
- Low haemoglobin
- Low vitamin D levels
- Hypothyroidism etc as important causes.

Bablu it’s very confusing according to what I understand u r father is suffering with Parkinson’s disease and is on treatment. 2 he has hydrocele. For ur information once hydrocele is detected it will never disappear until it is operated upon I probably think it might be hernia the way u describe
