I have seen the details given, from those it is clear that current treatment is not adequate. There is a need to upgrade. Apart from drugs, it is necessary to look at dietary pattern, extent of daily exercise, weight, height, lipid profile, bp readings, serum creatinine, urea and microalbuminuria.
In a well controlled diabetic patient fasting should be < 100 mg or closer to 100 mg, pp 150 to 170 mg & hba1c% < 7%. Thanks.

Lantus insulin is long acting insulin. If patients fasting/PP Sugars are not under control and and Hba1c is persistently above 8 % then some times it is advisable to go for premix insulin like humalog + NPH insulin for better glycemic control and maybe twice in a day injections. If you have recent post heart bypass surgery then it is very important to maintain Hba1c below 7%.
