Hello, thanks for the query.
I have seen the details mentioned. Usually at your age hba1c% recommended is < 7%. However, if you are maintaining < 6% hba1c% means the control is excellent. If you have no problems or no troublesome issues of getting hypoglycemic episodes (glucose going < 70 mg), then there is no reason to reduce the drugs. Maintaining a lower glucose level and hba1c% is quite helpful in preventing any possible complications associated with diabetes. But if you have any time en...more

Your doctor has taken right decision. Your focus should on controlling your father's diabetes to help him stay healthy not on whether he is taking tablet or injection. Insulin is usually required after long standing diabetes. But it's not true that once on insulin then you need it for lifelong.

Hi lybrate-user. There is no correlation between the sugars and hba1c. Hba1c of 6 if correct is too controlled and he may be facing hypoglycemic attacks. Need dose adjustment of medications. And creatinine is a but high may be landing in diabetic nephropathy. You can consult me online for diabetes dose adjustment and other issues. Thank you.

Hi Doctor, I am a 53 year old male, insulin dependent diabetic and a hypertensive. To add to this I am having BI polar disorder. Sugar control is average or below. BP under control. Now my medicines Lantus 20 units at Bedtime, Novorapid 10 units before lunch Glycigon 80, two times a day, two tablets each time For BP Olmin 40 LN Bloc 20 pantoprazole with Domstal one time a day As far as Bipolar problem is concerned I am cleared by psychiatrist to take all the above with the medicines he prescribed. I consider this a sensitive matter. I am always in stress because of my Job, I am the GM or second in command of a large turnover construction company. My morning sugar is between 95 tp 110, but post breakfast after 2 hours is 225. After taking my lunch I take novorapid 10 units hence for few hours my sugar drops to normal range. Again in the evening it when Novorapid effect is gone it climbs to 250-275. Remains like that perhaps till I sleep after taking Lantus 20 units, again in the morning it is normal. HB1C today's test shows a reading of 7.0. What should I do to take this reading to 6.0.

Mr. Lybrate-user, firstly, I congratulate you for having taken very good care of all those multiple problems you are suffering from. Secondly, I must commend the detailed manner in which you have described your present status. Thirdly, you have managed your diabetes pretty well. For your lifestyle & busy work schedule, an hba1c of 7% is quite good. You might be able to bring it down marginally, to 6.5% if you take care of your diet a bit more meticulously & exercise regulary. Beware of hypos whe...more

It is very difficult to give a clear answer why it happened. But it appears the earlier treatment of giving two sulfonylureas (SU) together was a wrong approach. Ideally one of the SUs is given along with metformin most often. Secondly diabetes itself damages glomerular filtering membrane in the kidney, this leads deterioration of kidney function. That leads to increase in creatinine level, protein excretion etc. In such situation Insulin is the best medication for blood glucose control. Ideally...more

As you are suffering from high blood sugar with high BP along with increased creatinine it is better to start with rapid insulin both every breakfast, lunch, dinner and at bed time lantus can be taken according to your blood sugar levels, after getting near normal sugar levels with creatinine levels then you can take tablet with minimal dose of insulin. This line of management will improve your overall general condition. Do not forget to follow timely food intake with reduced salt and oil along ...more

Good control of diabetes over time is the key to treating diabetic neuropathy. There is no cure for neuropathy, but keeping your blood sugar within a target range can reduce symptoms and prevent them from getting worse. To help control your diabetes, eat food that is good for you and exercise. Controlling diabetes means maintaining blood sugar levels (a1c) within the target range. This will do more than anything else to help prevent diabetic neuropathy from getting worse. Initial and ongoing tre...more
