Only ointment will not work. First get it properly diagnosed if it is fungal only. If yes then you need to take full course of oral anti fungal tablets.

Such resistant bacteria in a transplant patient is not a good sign.
Is he having any difficulty in passing urine or his urine is getting blocked?
He will require further evaluation, such as ultrasound of transplant kidney, urodlowmetry and if required few other test
may require reduction in his immunosuppressive drugs also.

Firstly you have to stop clobetasol cream. It should not be used on face.
To your query, it's difficult to say exactly how many days. As the steroid cream you have used would have damaged the face. So you need to consult a dermatologist.

Yes,The initial choice of treatment is a steroid ointment that is locally applied. (This can be suggested by a doctor after you are examined in person.)The ointment softens the foreskin and is applied for up to 6 weeks. When full retraction is possible the ointment is stopped.

If this treatment is unsuccessful, a circumcision (surgical removal of foreskin) is advised.
