As you r already having hypertension (though under control with medicines, you should b careful while using skin related chemical compounds. I would advise you to see any homoeopathic expert for your skin problem, where the treatment will b based on symptomatically, so you can get better results, I hope. Stop the first cream the betamethasone and continue for some more time with the sertaconazole and terbinafine, keep area dry and have bath atleast once a day, change clothes daily.

Swelling can be an abscess which is likely due to bacterial cause. You need to use antibiotic if so in proper course. If it is recurring can be a lymph node enlargement due to other cause. Better you go for ultrasound soft tissue examination and rule out the cause. Please don't neglect.
