you are very right about normal range of tsh levels. At 8.5 milli units/ml, with normal t4, the condition is classified as sub- clinical hypothyroidism. This normally is not treated with any drugs. It's cause of concern that your insulin is not acting to neutralize the sugar while you are taking good medicines prescribed by your experience doctor. This is non response of drugs. It's not fault in treatment by your doctor. I would suggest the support of ayurveda medicine as support to your current medicine. Both won't counteract with each other. when your teeth enamel is damaging you immediately need to take care of it.
Teeth have three layers, enamel-dentin-pulp. So when the enamel is damaged the dentin is exposed leading to weaker and caries prone teeth. See your nearest dentist and either to a filling or a crown/cap work done.

If he had a ear bleed do get a ct scan of temporal bones done to rule out any fracture. It's better to get assured than later repent. Keep ear dry no water should go into ears. Avoid using ear drops. Get also an ent consultation done to ascertain if there is any injury to the ear drum. Will need to take a course of antibiotics after the consultation to avoid infection.
