You have to adjust insulin doses after consulting diabetologist and switch over to other oral drugs since it is not working
1. Do regular aerobic exercise for 1 hour daily (brisk walking, jogging, running, swimming, cycling etc)
2. Reduce weight if overweight
3control diet
a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta

I was diagnosed with hypochondriasis 5 years back. Was worried about having heart issues and used to get tests done frequently as I was getting panic attacks and feeling anxious all the time. I was prescribed paroxetine 12.5 mg and clonazepam 0.5 mg twice a day and which was later reduced to once a day. Six months back my medication was stopped abruptly as I was stuck overseas due to covid. I also gained a lot of weight on paroxetine and have delayed ejaculation. Once the medicines were stopped abruptly, I lost around 7-8 kgs of weight but the anxiety came back. I was prescribed 100 mg of sertraline and increased to 150 mg per day but it didn’t work much even after 8 weeks. I am now prescribed 50 mg sertraline in morning and 12.5 mg paroxetine in evening to gradually switch to paroxetine again but I am afraid of side effects of paroxetine and also I have started getting panic attack. What can be other medicine with less side effects and can treat anxiety disorder. Thanks.

Received after 3 weeks. When these reports are normal, it generally suggests no organic involvement or a local lesion. The present complaints are probabably related to original psychological or functional disorder or at times due to multiple poly medication. She should be under treatment, supervision and follow up of a good psychiatrist or institute like nimhans for long duration.

Postpartum depression is the condition you seem to have initially had. In few women it continues as persistent depression. Zaptra is a good medicine for the same, but the clonazepam part in it needs to be tapered and stopped first and then if you are emotionally well the paroxetine part can also be tried to be minimised and stopped. For this, you need to consult an astute psychiatrist. The tapering of medications can take few months. All the best.

Paroxetine is a newer antidepressant known as SSRI. The dose you are taking is also low. There are side effects to any medications and this is why it is important that the psychiatrist monitors you regularly for both symptoms of depression and side effects of medication. However medications are prescribed after taking the pros and cons into consideration and when the benefit of medications outweigh any risks they may have.
