Most studies have shown effectiveness for 10 years in majority of people. Hence Tetanus vaccine is recommended once every 10 years, as long as one has received the primary doses of 4 vaccines in childhood.
A severe injury or large wound would warrant earlier vaccination along with use of Tetanus Immunoglobulin.
Minor injuries need not be occasions to repeat the Tetanus Injection. Definitely not 4 times in 6 months.

Yes the missed vaccine can be given as catch up vaccine. Give your child 2 doses at interval of 4 to 8 weeks followed by single dose every 5 years.
Best wishes

Better not to give. Better to give either on the same day or 4 weeks apart for the maximal effect.

You should yourself know by now if it is painless. Anyway it is not painless. Pain is a subjective phenomenon, experienced with different grades of severity by different babies.
