I am experiencing chest pain, discomfort on left side (front and back chest, and left upper arm). So far, doctors have performed Echo (clear, no issues indicated), TMT (when the technician stopped it, it showed Max HR 152 bpm 83% of max predicted 179 bpm. Max BP 160/100. Remarks say -" Good effort tolerance. No Angina or Arrhythmia. No Significant ST-T changes found), Endoscopy (didn't show any ulcers or any issues, normal mucosa), and Ultrasound scan of abdomen (only issue as per report is Fatty Liver measuring 17.5 cms). However, I have been to emergency room 4 times during last 1 month due to chest pain, sweating, etc. Everytime, they did ECG and Troponin tests and then sent me home after giving Pantoprazole injection. Doctor says I don't have any heart related condition. According to him, I have" Non-ulcer Dyspepsia" that is being worsened by my anxiety and panic. He has suggested - Pantodac DSR (twice a day), Atorvastatin 10 mg (at night), and Alprazolam 0.25 (at night). My cardiologist says that since TMT and Echo were normal, there is no possibility of a heart attack. I asked him whether I should undergo any further tests like CT Angiography to rule out any blockages etc. He told me very clearly that I am panicking for no reason and I should consult a psychiatrist. He says that my pain is result of anxiety and panic. I tried explaining to him that I do get anxiety and panic but it happens after I get chest pain, sweating, and breathlessness. What to do now? Should I consult another cardiologist? Is a clear TMT indicator that there is no risk of a heart attack? Is a clear Echo indicator that there are no other heart issues?

Dear lybrate user,
-you have undergone most of the tests & since they were normal the cardiologist has not advised further testing so do not panic & take the prescribed medicines, sometimes acidity also causes similar symptoms.
-engage yourself in various activities like walking for 20-30 min daily, yoga, meditation & other day to day activities.
-avoid spicy & fried food
-give sometime to the medicines to take effect

Lybrate-user: While levosulpiride can be used for stomach acidity, it is not approved for this use. It is actually an anti psychotic medication that can also be used for depression. It is sedating (makes the person sleepy) and increases appetite.

You should start to feel better within 2 to 3 days. It may take up to 4 weeks for pantoprazole to work properly so you may still have some symptoms during this time. If you bought pantoprazole without a prescription, and feel no better after 2 weeks, tell your doctor.

"As" per your clinical query is concerned pantoprazole 40mg once a day is enough but you can add any OTC antacid[Gelusil] to neutralize acidity in your stomach.

Dear lybrate-user, first of all we need to see why these medicine was started. Your symptom could be withdrawal symptom of levosulpiride but very unlikely. Wait for 4 to 5 more days for improvement. If no improvement consult psychiatrist. All d best.
