Hi doctor, I want to brighten my all body. My body color is brown I want to make full body white & want to stop hair fall. I have freckles on my skin. My age is 24, unmarried, weight 65 kg height 5'10" a dermatologist suggested me mevo tablets, I want to ask whether it will be beneficial for me or not, or you can suggest any other medicine. Yesterday, I jumped out of my bed half asleep for some emergency situation and have hurt my back. I applied Nanofast gel (Diclofenac Sodium IP 10 mg, Methyl Salicylate IP 150 mg, Menthol IP 50 mg, Absolute Alcohol IP 50 mg) yesterday while going to bed. Today when I woke up at normal hours I felt quite severe low-back pain while I went to wash my face at the basin. I took a capsule of Raceclo (Aceclofenac SR IP 200 mg, rabeprazole Sodium EC IP 20 mg) after tea. While writing this question the pain has subsided but I plan to take another Raceclo capsule at dinner tonight. Is it OK to take two capsules of Raceclo in 24 hrs. Is it OK that I am taking Aceclofenac Slow release capsule? If not then suggest me some other generic drug and dosage. Also please suggest some exercise after the pain subsides. I have a history of low back pain and have taken medication for that previously. I work in BPO/ITeS sector and had to sit for 8 hours at a stretch in front of computers.

Hi doctor, I am 41 y/m/ teetotaller. No diabetes, no hypertension and not on any medication. Medical history: have a sensitive digestive system with recurrent acidity/gastritis/ulcer kind of symptoms due to my sales/it job. Mostly manage thru diet and during flare ups, use ppis/antacids. Endoscopy and abdomen ct done few years back, which came out normal. So doctor suggested it could be ibs. Current issue: for the past couple of weeks, had a mild heaviness and a mild intermittent pain in the pelvic area. Consulted an urologist who suspected chronic prostate/cpps due to my sedentary lifestyle & has suggested rft, usg (kub and scrotum), uroflow, urine and semen culture to rule out. The ultrasounds were normal, urine and semen culture normal cbc – normal & most of the blood work values are also within range, except serum chloride which is 110 meq/l whereas the reference range is 98-107 meq/l. Creatinine is 1.02 mg/dl, serum sodium 148 meq/l, potassium 4.4 meq/l, bicarbonate 27 meq/l, uric acid 5.3 mg/dl and urea 19 mg/dl. 1.Is the elevated serum chloride a cause of concern & does it warrant any additional investigations? 2.Does these chloride levels suggest any kidney disease? 3.Since my pelvic discomfort is very mild & all investigations are almost normal, should I take any medications for this issue or just manage it thru exercise/diet? 4.How to protect and safeguard kidney from any future renal diseases? Sincerely appreciate your advice on the above.

You should try to manage your pelvic discomfort with lifestyle modification initially, if possible. Regular cold sneezing episodes could could be due to dns (deviated nasal septum) ent opinion and examination required. Your sugar levels are quite well controlled and metformin is first line drug for type 2 diabetes. You should continue that. Take it after breakfast instead so gastric symptoms will be less. Check your hba1c level. Try to keep it near 6.5. It is more reliable test.

My father aged 60 have sugar and BP. Now recently 4 days back he got bp of 210. Now he is hospitalised. Presently BP and sugar is normal. Blood urea is 109 mg/dl. S.Creatinine is 6.9 mg/dl. Sodium 126 mmol/L, potassium 3.5 mmol/L, Chloride 99 mmol/L. This is todays report. But yesterday S.Creatinine was 7 and 4 days back it was 4.3. As of now compared to yesterday creatinine is decreasing. Here doctors are suggesting for dialysis is this necessary. As we consulted other doctor they are saying dialysis is not required at this stage. Please suggest me. What to do should we take him for Bangalore.

Hello, it is difficult give an opinion from a distance regarding such acute conditions. However, if bp is near normal and blood glucose also in good contro, then by giving adequate fluid intake electrolytes can be managed. Same is the case with urea and creatinine. Best is to abide by the on the spot clinical expert, who can be the best judge. It may best to shift to a better equipped hospital as in bangalore.
