I am feeling with tooth pain on left side of face followed pain beneath the ear and then on forehead. It was a stobbing pain for a 1 hour and then it disappeared. Went to dentist, he had done OPG and then suggested COMBIFLAM as if there is no dentist medication need to be done. He had suggested it could be TMJ and SINUS and adviced for ENT consulatation. Went to ENT referred to CT scan sinus and spotted with mild sinutitus but the radiologist adviced me to have MRI. ENT doctor said i am suffereing from Trigemianal Neuraglia and gave MAZETOL tablet which was supposed to a tablet for FITS patient. WENT to Nero Surgeon ad adviced to take MRI 3T scan where again results turned with no significant of tumors or trigeminal nerve is normal. Dont know what to do but still enforcing pain at times after eating.I lost my taste after taking tablets not all specfically SALT taste and feeling nervousness at times.Having a big doubt whether the problem i am facing because of TMJ or TN.

I can understand your concern and anxiety after going to so many doctors. You need to be examined carefully and all your reports needs to be seen before suggesting any treatment. All the possibilities including Neuralgia, Sinusitis and TMJ problems are possible but the exact cause of the complaint in your case needs to be identified. I suggest you see us or any other well-trained Specialist at a good center. Please do not ignore your problem. Take care.

For your ready reference, I briefly describe my wife?s case. She is a 74-year- old housewife. She is a patient of chronic rheumatoid arthritis and osteoporosis and has been taking a steroid, wyslone 5 mg /day, for the last about 30 years and RISOFOC kit (calcium and vitamin D3) for osteoporosis for the last about two years. She has been taking other medicines also for hypertension (UDP 5 mg twice daily) diabetes (Glyson 5 mg thrice a day), high cholesterol (Zivast 20) etc. Now her BP and sugar levels are within the normal levels. She has terrible pain below the knee and above the ankle in the right leg and cannot walk. Her problem has been diagnosed as Rt side L5 radiculopathy and chronic steroid-induced neuropathy. An MRI done in Dec. 2014 showed L4-L5 Rt side foraminal compression. She has undergone a spine surgery on Dec. 29, 2014 and a transforaminal epidural injection at Rt side L5 level. Now she takes Lyrica 75 once every evening, tab Instrel ? thrice /day, tab Nexpro 20 twice /day, tab Rockbon once /day, and tab Mazetol 200 once in the morning, and tab Neurokind once at night. But there is no relief from the pain when she attempts to stand up and walk; there is no pain, however while she is in bed lying or sitting up. Please advise what can be done to alleviate the pain. Thanks for your attention and advice.

Relief on lying down.And pain present only on attempting to walk may indicate degeneration.In the knees or at the ankles due to prior arthritis. If possible do consult. Reevaluation may be of some help. Will try to help you in the best possible way.

Dear Doctor, Here I would like to give a short-brief of my son's problem of epilepsy in the following words. He got birth in a private clinic here in Delhi in the month of March 13, 2008. The delivery was normal and on the very next day of discharge from the hospital, he got a fits-like behavior and we immediately took him to the same clinic where he got birth and admitted there. As per the Doctor treating him it was said that his sugar got down and he was not needed properly during the night. After 3-4 days treatment, the doctor referred to another hospital i.e. St. Stephens Hospital herein Delhi and got admitted. After a week’s time the child got recovered. After this, about three years there was no problem with the child. However, after the age of 3, we could noticed that when child was walking he was complaining of some difficulty in walking on his right leg and immediately he asks to pick-up and also noticed a slight behavioural activity on his mouth. This is the starting point and we approached Dr. B.K. Gupta (Neurologist herein Delh). As per his presumption it was epilepsy and started treatment for epilepsy by giving the syrup Trioptal. By taking the above medicine continuously there was no problem for about three years. Unfortunately, after third year the child got fits again intermittently and we again approached the same Doctor and he advised to change the dose of the same medicine. But even though the duration of fits got extended but some time it happened intermittently. In the meantime we approached another Dr. (Prof.) Aldrin (Head of Neuro in IBHAS, Delhi). As per him we got the child’s MRI taken and after diagnosis he also advised to continue the same syrup Trioptal. In the meantime the syrup was not available in the market and the doctor advised to take Oxcarbazepine Tab (300 ml in the morning and 400 ml in the evening) plus Frisium 5 mg tab (one tab in the evening). Now for the last one and a half years there is no problem of fits however he has been complaining of tiredness intermittently, mostly after waking up in morning and also when he was about to leave for school and we presume this might be by taking the medicine continuously. Unfortunately, for the last 2-3 months we could notice another problem i.e. His right hand palm gets trembling (jerking) like feeling for a few second mostly every day and by rubbing and/or holding tight it gets normal and when it happens he is not able to move his fingers or hold anything, and also loose strength of his palm and it gets normal within a minute’s time. Earlier it was once in a week but now-a-days it happens almost daily sometime in the afternoon otherwise at school. Last week i.e. On 16/01/2017 we again visited Dr. Aldrin because of this above new problem. After check-up the doctor advised to get EEG report. However, as per the EEG report it was normal and the doctor advised to increase the dose of the existing medicine i.e. Oxcarbazepine 400 ml tab in the morning instead of 300 ml and continue the other medicine as usual. But even though the above medicine has been taking as per the prescription for the last two week the problem still persists. He has also the problem of Adenoid for the last 3-4 years and now-a-days treating by homeopathy medicine. At night when he sleeps he inhales/exhales through mouth and I am presuming that the above problem is due to this reason only. Kindly, for your advice.

You have not mentioned MRI report. It seems that the low sugar at birth might have left a permanent scar in his head and hence your child is having frequent simple partial seizures. Probably needs either adding a new drug or change of drug. Kindly seek advice from your doctor. Best wishes.

Dear, oxetol contains the active ingredient oxcarbazepine, which is an anticonvulsant and mood stabilizer. There are many long term side effects and you can read them on cims website. Regarding memory, there are nothing significant. Still you may consult your doctor for further advise. Take care.

Im searching a best seizure medicine which has very low side effect and which do not effect to liver, kidney heart and others organs, my neuro doctor has offered me to choose one medicine from "phenytoin, carbamazepine, oxcarbazepine, eslicarbazepine, sodium valporate, topiramate, levetiracetam, lamotrigine, diavalporex, phenobarbitone, currently i'm taking clobazam 12.5 mg and lacosamide 200 mg per day but nothing improvement. I feels suddenly extreme difficulty till 30 to 50 second, I always remain conscious at that time and can talk with anyone but that difficulty what I feels which is unable to express in words. My mri report show: ring shaped cortical/ subcortical lesion with eccentric dot and blooming on swi and calcification on phage image in the left hippocampal formation with minimal peripheral gliosis/edema. Findings represent calcified granuloma most likely related to neurocysticercosis. Other diagnostic consideration is tuberculosis. 2. Mild increased signal and volume loss of the left hippocampus as compared to right. Finding are suggestive of left mesial temporal sclerosis. Clinical correlation is suggested" while I do eeg test when normal condition always show normal eeg but when 30-50 second extreme difficulty happen during eeg show abnormal ieds: left anterior temporal and there are few sharp wave discharge starting from left anterior temporal region. Now according some above report which medicine is the best for me please suggest. I want to change medicine lacosamide and clobazam. Thank you.

Every medicine affects some organ systems. As they get metabolised or excreted or turn into active metabolite. Levetiracetam does not affect liver and is well tolerated but it does negatively affect kidney function.
