Mr. lybrate-user, Thanks for your query.
The drugs mentioned Glimepiride and Voglibose are different in their action.
Metformin is supposed to reduce or delay glucose absorption and hence slows down blood glucose rise after taking food. Plus it also reduces insulin resistance hence helps blood glucose control. There are other beneficial effects like helps slight weight reduction.

Hi,
presently you are on triple drug therapy with optimal dose of each medicine.
As your sugar after food is 260 it seems that more tighter control of blood glucose levels is needed
the exact blood glucose for you may differ according to your age and associated comorbidities.
But before increasing your medicines it is necessary to know whether you are taking present medications in correct timings
voglibose has to be taken 15 to 20 minutes before food

lybrate-user, thanks for the query.
For a person with diabetes it is very important to keep the blood glucose in strict control, otherwise, the long term complications associated with diabetes affecting eyes (retina), kidneys, lower limbs, peripheral nerves and heart can occur more rapidly. Ideally fasting glucose should be < 100 mg, PP 160 to 70 mg & HbA1c% <7.
Missing a single dose of Voglibose does not cause any problem. Voglibose is to be taken either with meal or just after it. Sinc...more

fasting should be around 100
slight fasting is high.
medications are fine.
complete your dinner timely
take less carbohydrate in dinner.
if possible check mid night around 3-4 am blood sugar
to see the level at that time .in many of case when sugar level gets low in midnight than compensatory mechanisms happens in body and blood sugar rise leads to high fasting value .
in this situation we need to titrate night medication and diet properly

Mr M D Agrawala, Thanks for the query.
With HbA1c% 10, it is clear that diabetes is not in control. That is the reason Insulin injections have been added in the treatment. There is no mention as to from when is on the insulin injections. Plus it is also necessary to know her serum creatinine & urea levels, to look at the possibility of reducing or discontinuing Insulin. Unless blood glucose level is strictly controlled, it is not possible to discontinue Insulin. Thanks.
