Please explain report of FNAC report what aaya what stage of cancer Clinical History:-Obstructive jaundice. MRCP: Suggestive of carcinoma GB with peripheral lymph node comprising CBD causing obstructive jaundice. FNAC Site:- EUS guided FNA from two sites: 1) Lymph node. 2) GB mass. Gross Description:- Ten giemsa and seven pap stained slides were examined. Microscopic Description: 1) Hemodiluted smears show a polymorphous population of lymphoid cells with occasional histiocytes and sheets of benign epithelial cells. 2) Hemodiluted smears show cell clusters with nuclear crowding and overlapping, moderate nuclear pleomorphism and hyperchromasia with singly scattered atypical cells and sheets of benign epithelial cells and occasional histiocytes. Final Diagnosis :1) EUS guided FNA from Lymph node:- No definite atypical cells seen in smears examined. 2) EUS guided FNA from GB Mass:- Features are suggestive of Adenocarcinoma. Advised:- Histopathological correlation.

Dear Mr. Manish,
From what you have explained it appears to be a locally advanced Ca. Gall bladder. Usually they are very aggressive . I would get a PETCT done, if its only locally advanced , a plastic stenting needs to be done ( through ERCP or PTBD). this should be followed by 3 cycles chemo and reassessed. Hello doctors, this is my USG REPORT taken on 17 February 2018 LIVER: Normal size and parenchymal echo texture. There is no focal lesion. There is no intrahepatic biliary dilatation seen CBD: Normal and non-dilated GB: Normally distended. No intra-luminal calculus seen. There is no abnormal GB wall thickening or peri- GB collection seen. PANCREAS: Normal size and parenchymal echo texture. MPD is not dilated. There is no focal lesion. SPLEEN: Normal size. No focal lesion seen. RIGHT KIDNEY: Normal in size and cortical echo texture. Normal cortical-medullary differentiation is maintained. No hydronephrosis or calculus seen. LEFT KIDNEY: Normal in size and cortical echo texture. Normal cortical-medullary differentiation is maintained. No hydronephrosis: Two calculus seen at upper pole of kidney measuring 0.41 and 0.37 cm URINARY BLADDER: Normally distended. A intra-luminal calculus seen measuring 0.96 cm. There is no abnormal wall thickening seen. INTERNAL ECHOES ARE NOTED PROSTATE: Normal size and shape, Normal echo texture, calcification noted IMPRESSION: LT. RENAL CALCULUS WITH UB-CALCULUS WITH UTI. Is this very serious? What should be my next step? Please HELP.

I have gone through the details mentioned. The skin infection (boils) can recur if complete eradication doesn't occur. The antibiotics that you have been on may not as effective on staphylococcal infection. Plus there is a need to have a stricter blood glucose control. So please look at these aspects. Prostate size does not matter until and unless associated with lower urinary tract symptoms. Do not go with the report, you may need medicines if you develop symptoms due to that enlarged prostate. Sometym patient with prostate size 80 cc may not have any symptoms and few with 24 cc have severe troublesome symptoms. So do not focus on what report says. You can go for surgery for gall stones.

For gall stones. Plan your surgery electively. It is usually routine laparoscopic surgery with 1 day stay in hospital and recovery in 5 to 7 days if you r working. Should get it done whenever you have the time and as complications are not good.
For kidney cyst. Nothing to be done.
For kidnuy stones. Also nothing to be done.
