If your pain is more in the distal joints, ie. In the upper limbs if the pain is present in the fingers/wrist and in the lower limbs it the pain is present in the toes/ ankle, then we shall definitely say it is Rheumatic arthritis. Cervical spondylitis is common at your age. By changing medications might not help you much. Ultracet will cause nausea, giddiness too.
You need more physio and safe medications.
Hope he did MRI scan, to see weather anything can be done surgically. Otherwise injections to neck will be much helpful.
