Common causes of anal fissure include: Passing large or hard stools. Constipation and straining during bowel movements. Chronic diarrhea.
Externally applied nitroglycerin (Rectiv), to help increase blood flow to the fissure and promote healing and to help relax the anal sphincter. Topical anesthetic creams such as lidocaine hydrochloride (Xylocaine) may be helpful for pain relief.

You need to be examined by a general surgeon and proctoscopy to be done to find out what exactly it is, if it is a fissure then keep your stool soft by taking plenty of fluids orally and high fibre diet even you can take syp cremaffin 3 to 6 tsf hs daily, use ointment lox anally before defecation and ointment nitrogesic anally qid for 6 months, proper treatment follows according to examination.
