Asked for male, 62 years old from Thane

I had a general pathological check up. I am 57 and have controlled BP of 127/80 on an average with the medications concor5, losacar50, nicardia retard10, ctd12.5 (1/2 tablet) and neuroboneforte all taken in the morning followed by Rousavas10 and nicardiretard10 at night. My report shows Hba1c 5.8,mean plasma glucose 119.8,vit d 26.87,b12 at 730, bun at 5, sodium at 132, urine sample shows Protein (+) rbc detected 2.all other reports like uric acid, potassium in control. I do have unnecessary anxiety and not able to control my emotions hence get upset regularly. I walk daily for around 40 mins but also feel floating sensation apart from ankle pain that is regularly there. Wanted to know if the parameters are ok and any other suggestions.

Vit. D is low start its supplement. Urine shows RBCs which needs further investigations. Please consults a Physician with reports.

Good morning Dr. I am a 43 years old african american woman with a personal hx of htn (i had a seizure last year last year, due to sleep deprivation and stress. My neurologist has me taking keppra 500 mg bid. I haven’t had any additional seizures). I also have a personal hx of high cholesterol and my pcp has me taking the following meds: losartan, carvedilol, folic acid and nifedipine. My question: would I be a good candidate for drinking nettle tea? I am originally from the island national of haiti and often use herbal medicine/ remedies to treat and my previously mentioned conditions. Any feedback that you can provide would be greatly appreciated. I of course plan to discuss this with my medical providers before taking it but I wanted to get your professional opinion. Any information that you provide would be greatly appreciated.

Hello,
though the exact details are not known but it does have some effects if taken with some antihypertensive medications reportedly. As you are taking these medicines Please do not consume it without taking clearance from your doctor

My age 35, male. I have anal fissure problem since two months. I have used pilex tab & pilex forte ointment, but not cured anal fissure. My anal muscles (sphincter) very tight & outside anal muscles swelling there. Swelling not decreasing. Pain while going to washroom. How can I relax anal muscles (sphincter) (means loose anal muscles) present I am in africa. I have ointment cream gel (diltiazem hydrochloride 2 % w/w) escot cream (nifedipine & lidocaine cream) diltigesic organogel (diltiazem 2 % organogel) in this above ointment cream, which one relax anal (sphincter) muscles & reduce swelling how can I use this cream, can I put cream only outside anal muscles or even inside anal area can I put within how many days, can I cure swelling & relax anal muscles please answer fast thanks & regards waiting for your reply.

Hello... nefedipine + lidocaine combo is good..
use it thrice a day. Apply it all around with your finger and also push a bit inside with your finger. Do warm sits baths for 10 minutes before applying it.
Also avoid constipation..

Sir, I am 33 years old doctor by profession in critical care. Recently diagnosed to have htn and hypothyroidism since one month. Initially, bp was 190/110 now it is under control. Initially started with labetalol but I am very allergic to that had severe palpitations dizziness and itching for that. Methyl dopa no manufacture. So used olmezest beta. Now under control but changed to nicardia retard as it is safe to conceive. But I have developed oedema for it. Kindly suggest an alternative medication to control my htn. Tsh is 10 and anti tpo is 1000. Started on thyronorm of 150 mcg since 1month. Did complete workup for pcc. All reports normal. Usg and showed mild PCOS and renal doppler normal. Any suggestions for further work up.

Would say if hashimoto you have with cystic degeneration then pulse therapy would be needed well thiazides are another option.

I'm 37 year old male on bp meds since the age of 29, i've been on atenolol 50, nifedipine 10, amlodipine 5, currently on metoprolol succinate xl 50 and cilnidipine 10. I learnt from my personal experiences and also from web literature that beta blockers/calcium channel blockers can cause slow heart rate or bradycardia unlike angiotensin receptor blockers (arb' s) like telmisartan or thiazide diuretics such as hydrochlorothiazide or chlorthalidone. Can I switch from beta blockers to arb's with docs advice to negate the effects of beta blockers. What about celiprolol, carvedilol and pindolol? My last bp readings were, 130/90, 140/80 and 130/85 monitored every 7-10 days. I have undergone my lipid profile, kidney function, liver function and thyroid tests a month ago and everything seemed normal. I'm not diabetic and it doesn't run in the family as well. I do have allergic rhinitis from so many years for which I have tried levocetirizine, montelukast and fexofenadine meds as and when required. Also take placida plus for anxiety issues. I'm just asking this out of curiosity.

What is best for you your doctors know better than you as the beta blockers which you have mentioned you have just googled them and have no idea regarding their efficacy potency or mortality benefit. As matter of fact you should not smoke weed as it has very serious side effects and definitely not once you are a hypertension patient taking nifedipine.

Hello, Thanks for the query.
I have seen the details given. I learn that you have Hypothyroidism and hypertension. But there is absolutely no mention of necessary details like T4, T3 & TSH levels, BP readings, dosage of Levothyroxine being taken and also dosage of other drugs taken. Without any of these details, it is difficult to give any opinion.
In pregnancy first choice drug for hypertension is Methyledopa, then Labetolo and Nifedipine. Olmesartan is contraindicated.
