You should probably have a scan of your kidneys and consult a nephrologist to make sure that there are no renal causes apart from diabetes. Telmisartan can transiently change creatinine levels but it protects the kidneys. How is your blood pressure. Try to consult a doctor.

Your blood pressure is quite high despite taking telpil (40) od. You should at least get some investigations to rule out secondary complications of hypertension in form of cbc, rft, rbs, lipid profile, ecg, echo, fundus examination. Now for your bp control, telmisartan is protective for kidney but upto certain limits. It is not advisable to take it with significant kidney disease as it can cause increased potassium.
