You could try an alternative brand of mometasone spray or saline nasal drops. To get a permanent solution you could get an allergy test done by skin prick method and start immunotherapy.

As you have already tried a course of local steroid, it's better to proceed for surgical removal of both adenoid and tonsil.

You should get nasal endoscopy done to rule out any nasal pathology causing nose block and any mass in nasopharynx causing eustachian tube blockage and ear problem. Do steam inhalation twice a day.
