Hi, this is because of allergic rhinitis.
Take tab. Septillin 2-0-2 after food and rhinorin lehya 1 tsf twice daily followed by milk for 3 months continuously. Take imunocin 1 tab. Twice a day for 3 months to gain immunity in the body. Avoid cold and refrigerated food. Avoid sweets and oil fried food. Perform pranayama daily in the morning for 10-15 mins. Avoid exposure to dust by tying a kerchief or a mask around the face covering the nose.

There are many reason for bad breath. If mouth wash is not effective then it may be due to nasal crust or nasal infection.
Try this-
tab cefixime 200 mg twice a day
tab cetrizine 10 mg once before sleep.
Nasal douching thrice a day.
×5 day
avoid if you have allergy with any of the above medicine.

Even though Ayurveda is an established medical science but I have some reservations for advising you to follow it for your kid. More so because she has a history of pneumonia. Please visit an ENT surgeon or pediatric surgeon for the definitive treatment of nasal polyps. As far as I know the nasal decongestants are not advisable for kids, at the most the Nasoclear is saline based nasal drops you can use for her along with steam inhalation.
