Consult a dermatologist in your nearby area for definitive diagnosis and treatment. Also, ask your family members or roommates if they have any history of night itching or any other skin lesions.
Hope this helps.

Hi lybrate-user, please stop the ointment which is having steroid (methadone) in it. It is not advisable. For piles, you need to take care of your constipation and straining. Point sucral ano twice daily is often helpful. Also to sit in warm water and take a laxative. But most importantly, please visit a surgeon nearby to confirm your diagnosis, not any other disease.
