You need to get some basic investigations done i.e. A scrotal ultrasound along with color doppler of testes and cord structures following which you can be operated upon.

Finding 2 pus cells in urine is not an infection and you have taken the antibiotic pill unnecessarily and it can cause side effects as you have described . No need to worry and and be normal

No, I do not think so, ofloxacin is not know to cause gynecomastia. Besides for an adult its only half the dose. And even if so any medicine causing it once withdrawn the effect gets reversed in the majority.
