Dear this itchy patches may be because of some fungal infection. You can try applying candid cream twice a day and apply some talcum powder to keep the area dry. If it does not help, then you hv to visit the doctor personally.

As such there is no medicine which can revert the kidney damage. You have to strictly control your diabetes and high BP that will be helpful in preventing or slow down of further kidney damage.
Avoid nephrotoxic drugs specially pain killers and NSAID.
Monitor kidney functions regularly.
