You should immediately stop this cream. Primarily because it doesn't suit your skin as it contain tretinoin & secondary reason is because it contains a potent steroid in it which is not good for your skin.

I am sorry to hear about your concern but will be happy to assist you.
Sun exposure and darkening of the skin due to a burn or any other injury can also cause dark patches around the lips. In very cold weather, if you lick your lips and the area around it too often, it can discolour the skin and make it look darker.
Let's connect over a call so that we can discuss your concern in details and make a suitable treatment plan for you.

You may stop using the cream for a couple of weeks and see if the white patches improve. If they are persistent, it would be best to consult a dermatologist to evaluate whether the patches have appeared as a result of using the cream or the underlying cause is something else altogether.

If you don’t wash your penis every day, a cheesy-looking substance called smegma can build up. Smegma is a natural lubricant that keeps the penis moist. It’s found on the head of the penis (glans) and under the foreskin.
If smegma builds up in the foreskin, it can: start to smell. Homoepathic treatment cures it without side effects. You can click on consult option for homoepathic treatment..
