Hi! the majority of symptoms that you have measured overlap between the two. Like obesity itself can result in stretch marks, round face, lethargy, resulting in altered menstrual cycle. Obesity in itself is major cause for pcos. But since you've mentioned that 11-12 years back your cortisol levels were altered then it is better that you get your cortisol and acth-dexamethasone suppression test. And in addition get thyroid profile also done.

Stop thinking about adrenal cancer. Keep faith in your doctor. If reports are coming normal that proves you don't have such problem. Abdominal pain is explained by your cortical cyst it presents with abdominal pain generally. But treatment is symptomatic. You need to confirm your low cortisol levels with an overnight dexamethasone suppression test or a 24 hr urinary cortisol or a midnight salivary cortisol levels. If it is really low, then it has to be worked up properly to find out the cause & treated accordingly.
