I am a 30 years old male (Married), having below chronic symptoms: - Getting anxious and Rapid heartbeat rate - Excessive dreaming during night sleep and getting up exhausted in the morning - Mood variation from low to high and vice versa (less frequent) I have been on treatment since 2004 till present. I have been under treatment of several psychiatrists (Due to studies and job posting my current city kept on changing) and got to know below terminologies as part of their diagnosis in chronology of consultation: - Subconscious state depression - Endogenous depression - Bi-polar disorder - Depression & Anxiety combined - General anxiety disorder - Latest diagnosis - No anxiety, its Insomnia Tests: Have been through EEG, thyroid profiles, Liver & Kidney profiles etc. None of the above test was concluded to be the cause of my situation by the psychiatrists I was consulting. Currently all the anxiety / depression medicines stopped by my psychiatrist. I am on amitriptyline 10 mg and Melatonin 3 mg. None of the treatment seems working. My both the parents also have some psychiatric problems but they are stable with their current treatments. Happy atmosphere at home. Can some help me out of this situation. I don't find a root cause of my symptoms so that it may be worked upon. Its really disappointing and making me hopeless. Thanks.

I am psoriatic arthritis patient with back pain and si joint pains in addition to other joint pains. I use hcqs 200 mg for psa. Spine surgeon prescribed duloxetine 20 mg one in morning & one in the evening for my back pain. These tablets also reduce other joint pains and also helps in controlling my inflammation. But my problem is because of this duloxetine capsules my sleep has become very much disturbed and I am unable to fall asleep when I wake up in the night. I feel active all the time and suffering with racing thoughts/brain fogg. I am not able to calm my mind by all means. To counter this I added mythy cobalamin 750 mcg and 1/2 of clonotril - .25 mg. But still then I get up between 3 to 4 am and unable to fall asleep. Totally I am getting a sleep of about 4.5 hours to 5 hours a day. Increasing clonotril dose causing me irritation in the stiff muscles surrounding the joints. I tried magnesium supplement which worked nicely but it increased my scalp psoriasis rapidly. I tried all forms of magnesium but same results. Melatonin helped a bit but it causing tiredness during entire day. Could you please suggest any medicine to calm down my thoughts and to get a restful sleep? Thank you.

-if duloxetine is causing so many problems better to stop it - start on drugs methotrexate hope you should be recieving for your psoriasis the details of your psoriasis treatment is not available here - if you can upload the treatment details of psoriasis I can help you in better way.

Hi, doctors. I really need your help with something. So my boyfriend he is 23m now. He is on paroxetine 50 mg, melatonin 10 mg and propranolol 40 for anxiety. For past 1.5 -2 years he keeps developing this very severe pneumonia episodes constantly. He would get a mild cold. He'll take montelukast, levocetirizine and it'll be almost unnoticeable. Just a runny nose sometimes, you know the kind you get if you have a dust allergy or and a sneeze here and there. But then within a few weeks he'll suddenly develop phlegm in chest, severe coughing and severe shortness of breath. And then he'll require antibiotic treatments. Most of the times 2 of them together and cough syrup, bronchodialaters and what not. The frequency of this is concerns me. The last was in june last week I suppose. He then again developed this today. Like a month probably goes by and he gets these. I am just so worried. Like could there be something wrong with him! why does he keep getting those! all the time. A lot of the times there even have no clear triggers. This time he got wet in the rain though. I don't know what to do and i'm worried out of my mind. Any advice is appreciated. P.s he has no other illness that I can think of. Although the gp was a bit concerned about his neutrophil count in june. Then he repeated the test and the value was 43. Then gp said it's all ok in that case. Not any smoking drinking or any other substance is done.

Do a pft spirometry pre and post mdi test proove that is small airway disease or bronchial asthma treat accordingly with nebulisations and mdi l pump and medications for the same.

Melatonin is over the counter in countries like us, it is prescription drug in india. For short term like few days you can take, on longer run if prescribed you need to consult a psychiatrist, as it needs to be monitored for liver function test, etc. It can cause mild constipation, as a common side effect. You should take any medicines with only clear advice of doctor your body likely produces enough melatonin for its general needs. Melatonin can be used to treat delayed sleep phase and circadian rhythm sleep disorders in the blind and provide some insomnia relief. Treat melatonin as you would any sleeping pill and use it under your doctor's supervision.
