Based on the details mentioned it looks like her blood glucose level is not in good control. It is better if her HbA1c% is checked, which gives an idea about long term control. Usually in patients with long standing diabetes in later years, treatment with Insulin becomes a necessity. Because oral drugs do not give necessary response. Yes you can take insulin 10 units before dinner but which insulin it is humalog 30/70 but please check your sugar levels regularly for more information contact at my clinic or consult privately.

Kindly try and analyse your life style and see if you have started eating may be fat based or protein based food items more thinking they are not sweet and can be safely consumed. Reapeat your HBA1C with change in diet on current medicines.

I feel the medications that you are taking are right. Without knowing your blood glucose levels it is hard to determine the exact dose though.
K GLIM M-1, Ten DC 20 mg, Voglibose 0.3 mg and Piopod 15 are for your diabetes control. Prolomet is for BP control and Ecospirin-AV is to thin your blood and control your cholesterol levels.

During initial period of diagnosis, insulin helps to bring down the sugar rapidly and to reduce your problems fast. Later the insulin can be switched to oral tablets. Now the usage of tablets will be less if you can stick to your diet and exercise.

Blood sugar low is not desirable specially 42.
Reason for low blood suagr is too many medicines and insulin. Another cause may be inconsistent ,irragular dietary habits. Irragular exercise imay be another cause.
Look into these points and try to correct them with the help of your treating physician.

Based on that it is not possible to advise any change of treatment There is a need to know the following details : Fasting, PP glucose levels, HbA1c%, dietary pattern, extent of daily physical activity (exercise), lipid profile, BP readings, serum creatinine and urea. Your dosage needs to be adjusted sir you have to take some diabetic drug at lunch also to bring down the post lunch sugar kindly consult your diabetologist or else you can consult me.
Sugars should b controlled to prevent diabetic related complications.

"As" per your clinical history is concerned please discuss with your family doctor about the nature of your disease [Type 1 and Type 2], if type 2 then you can take medication as mentioned and if type 1 then you can take insulin regularly. In any case sugar level, 312 is very high at your age and it can deteriorate your heart further.
