Hi lybrate-user, you have not mentioned your glycemic status (hba1c) ,so status on your glycemic control can't be commented. Urine for microalbuminuria tell about failure of your kidney to retain small protein and not let them lost. Remember 1st thing is good glycemic control.
2nd hypertension to be rulled out.in case of hypertension ,special category of drug arb / ace-i be initiated but only after evaluation by your physician. I suggest you to consult your diabetologist.

Dear Mr. lybrate-user, as per your symptoms indicate, it can be a muscular spasm pain. However without a thorough examination, nothing can be conclusively said. You can continue taking the medications you have mentioned. If giving inadequate relief, short courses of muscle relaxant and exercises can be given. Still if pain persist, interventional pain management options can be tried where in nerves carrying pain from the painful areas are blocked.

You must first consult orthopaedic surgeon, on his advice have x-ray done for cervical and thoracic spine ap and lateral. He may also advise mri. In the mean time he will prescribe you muscle relaxants and hot fomentation and muscle relaxant gel. Get a cervical pillow and if the doctor advises than you can go to physiotherapist for traaction, short wave diathermy and tens but do not do any exercises for neck. Cervical pillow may be advisable for you.
