I have gone through the details given, however, there is no mention of actual glucose levels like - fasting & PP and HbA1c%. If a person has Type! diabetes then only Insulin injection is given. she is taking the best regime of insulin. The only option left is now to go in for a insulin pump. But please meet a doctor or a diabetic educator who can guide you for insulin adjustments. You should know carbohydrate counting and insulin sensitivity factor (isf) to adjust your doses and take corrective doses of insulin.
As for fasting try increase the dose of lantus to correct the fasting. I hope lantus is taken at fixed tie everyday.
Try to do some exercise like walking lightly...more

Actarapid is a human insulin with rapid action. Apidra is also a rapid acting insulin but the compound is different. Actions are similar as both have rapid action and that stays for 4 to 6 hours. Both are used to control after food glucose rise. Dosage can be similar. Thanks.

Actrapid alone once daily is not the right Insulin for your condition. Actrapid acts for only 8 hours and instead of Actraid, Mixtard 50/50 twice daily is more suited for you. Go to a nearby general physician and he would start you on Mixtard 50/50. Start with 28U 30 min before breakfast and 24U 30 min before dinner. And adjust the dose accordingly and the right dose would be understood in few days.
