Hello, thanks for the query.
I have gone through the information given. You have mentioned only the fasting glucose level. There is no information on after food glucose & hba1c%. Also there is a need to know her lipid profile, serum creatinine & urea levels to give any specific advise. It is better if you also give information on her dietary pattern, weigh, height and extent of daily exercise. Ideally at her age fbg should be < 100 mg, pp should be around 150 to 160 mg & hba1c% < 6.5%. That ...more

Mr. lybrate-user, thanks for the query.
I have seen the details given. Since there is no information on fasting, pp glucose, hba1c%, lipid profile, dietary pattern & extent of daily exercise, there is no way to give any suggestion. Secondly your bmi (body mass index) is 28.65 kgs/sq meter (normal < 23 kgs/sq meter) which means you have an excess of over 15 kgs. This is a major hindrance in better glucose control. Plus I do not understand the reason for changing the medication. Please come ba...more

If only metformin is taken it should be after food. If sustained release tablet is there then od may ok. If not twice a day is better. Side effects are less if taken after food.

Do lifestyle changes. Follow diabetes diet which simply means eating the healthiest foods in moderate amounts and sticking to regular mealtimes. A diabetes diet is a healthy-eating plan that's naturally rich in nutrients and low in fat and calories. Key elements are fruits, vegetables and whole grains.

Avoid sitting Cross legged.
Avoid Squatting-
Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.
Core Strengthening Exercise- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day.

Hi Mr. lybrate-user! as you have chronic pancreatitis, I would advise you not to take sitagliptin and other drugs belonging to that class, i.e, dpp4 inhibitors. Good that you stopped it. As the sugars, especially fasting bloood sugars are high inspite of taking a high dose of tablets, you may consider taking insulin at night which will bring down the fasting value.

Your relative had suffered cv stroke most probably due to uncontrolled diabetes as evident from your description.
If treatment started within one hour of onset of stroke symptoms reverse in most of the patients.
As your doctor has advised speech therapy and physiotherapy means stroke features can not be reversed.
Further improvement will depend upon control of all risk factors like diabetes, hypertension, dyslipidemia etc. And effective speech therapy and physiotherapy.
