Istamet is not the right type of medicine to de-escalate since you are having such a good sugar control by diet and exercise. The correct way forward should be to stop istamet altogether and just give you Metformin (which is present in Istamet). I believe you would do well on Metformin alone.

Ir is immediate release and sr is sustained release formulations. Both work well but usually sr formulations have lesser gastrointestinal side effects.

Hello, thanks for the query.
Both the products are same. The er refers to extended release, which means the drug is released slowly over 12 hours in the gut. That helps maintain a longer therapeutic level. It is beneficial more so if a person has any side effects like gastrointestinal disturbances if plain metformin is taken. Both are fine. Thanks.

Most of the patients whose HB1c is more than 10% with blood sugar in range of 500 mg I always initiate with insulin therapy and continue insulin for 3 months till Hb1c is less than 7% and continue control of sugars with the help of suitable oral tablets.
