Asked for male, 70 years old from Bangalore

Doctor, I am having dm for 20+ years. Hypertension for same period. The combination of amlong 50/5 + monotrate 20+ clopidrogel + telmisartan 40 has controlled hypertension very well. However dm control is not satisfactory with metformin 1gram + glimepiride 4 mg with breakfast and galvus met 50/500 + glimepiride 4 mg at dinner. Of late fasting is 130+ and pp is around 240. I am 70 years old, walk for 60 minutes on treadmill 4 times a week. What change can I do to control the dm part please (my hb1ac is 7.5). Want to avoid insulin as far as possible.

Monotrate is for coronary artery disease, and clopidogrel is again a blood thinner used for coronary artery disease, and these are not drugs for hypertension.
You require a complete evaluation for coronary artery disease incuding an ecg, tmt, 2d echo and a coronary angiogram depending on the results of above tests. Visit a cardiologist nearby.

Dear sirmy father current age having 64, in 2016 did bypass surgery. Now he taking allopathic medicine 1) ctd m 50/6.25 2) monotrate sr 60 3) rozucor gold 10 4) remylin 5) folvite. Sir, I am a 60 years old having hypertension patient since 1987 and type 2 diabetic since 2010 without medication now but under control. HB1C last was 6.3. In March 2017 I suffered for a month from irregular angina pain when on advice of a local Dr. I rushed to Bangalore where on angiography was diagnosed 90% blockage in main LA, and on advice from cardiologist got don't Angioplasty/PTCA, STENT fixed. 16 months are over and occasionally I feel discomfort which my Dr. suggested is due to my stressful/tense nature. Medicine still continuing are. METLOR 25- 1 tab after breakfast for BP PLAVIX 75- 1 tab after breakfast ecosprin 75- 1 tab after lunch olmesartan 20- 1 tab after dinner for BP ATORVASTATIN 20- 1 tab after dinner (LDL/HDL under control) NITROFIX SR 30- 1 tab after dinner (cardiologist on follow up after 12 months advised to continue all) Sir, I am veg, non smoker hardly 4-5 hour sound sleep, continuing with morning brisk walk for an hour without discomfort for 4 to 5 days a week.

My father is suffering from cystitis with hematuria, since 3 months. His ct scan report is as follows- 7 mm calculus is seen at rt Vesuvius- ureteric in bulging in the bladder lumen. It is persistent at its rt Vesuvius- ureteric jn even in prone position. No proximal hydronephrosis or hydroureter is seen. He was in LVF & had an acute coronary syndrome 3 months back. No coronary angiogram was done. He had also suffered a pulmonary emboli in July 2016. At present he is on following meds-flavedon mr, monotrate 30, lasix 40 mg, 1/4 tab, cardivas6. 25 mg & clopilet7 mg. For his UTI he has taken fosfomycin 3 mg & Zifi-cv (200) mg. Kindly advise if we can consecutively treat him for his calculus as we are against surgery considering his cardiac status & age.

Spinal canal narrowing at L4-L5= 6 mm, L5-S1=8 (other L1-L2-L3=11) REPORT N.C.V both lower limb-Bilateral, Sensory mother Axonopathy of all four limbs, Correlate clinically. PLEASE help.

I am 83 years old army veteran, about 83 years old sikh. Suffering from scalp dandruff (white scales falling while combing, redness of scalp skin, frequent itching, at times resulting in scalp skin injury/slight bleeding for short time) for the last 30 years 9 months treatment at bakson homeopathic clinic (cost rs. 14,000/-) did not help. Army hospital alopathic treatment withpropysalic (clobetasol propionate) lotion, desonide (dosetil) lotion, urea &lactic acid cream, ketoconazole lotion, scalp shampoo, candid tv shampoo, coal tar shampoo (from usa ), heads & shoulders shampoo (used for 4-5 yrs), pnatene shampoo (used for 4-5 yrs) has not helped. Now trying ketoconazole and zinc pyrithione lotion (kz plus lotion), mometasone furoate & terbinafine hydrochloride topical solution. Dandruff has resulted in thinning of hair. Preset suffering from osteoporosis arthritis and taking me ficinies like ca, coq 10, glucosamine chondroitin, bifos e5 (alendronate sodium) etc. I had undergone cabg surgery about 22 years ago and taking presently monotrate 10 mg bd, aspirin 75 mg of bp medicine diltiazem hydrochloride 90 mg tab od. Request advice to get rid of this problem.

I am an old of 73 years. At present feeling a respiratory trouble & a mild pain on left chest, most probably due to 1st stage of Angina. I take medicine for BP e.g. Espin-AT & Losium-50 and for ear blockage I take Trinerve Cap, Gincoba Tab, Relent Tab. What should I do for my chest pain? Please help me.

Get an ECG and Trop i done. If any positive consult a cardiologist . If chest pain related to exersion then idealy you should go for CAG to know status of coronary artery. You can add monotrate 20 mg once/day and se response ,if not intersted in any intervention.

I need the second opinion for my old aged mom (almost 95 years). She is suffering from all these symptoms those are mentioned below. Loss of appetite means she doesn't have the same desire to eat as she used to. Signs of decreased appetite include not wanting to eat, unintentional weight loss, and not feeling hungry. The idea of eating food may make she feels heartburn, gastroesophageal reflux disease (acid reflux) as if you might vomit after eating. She is mostly indigested whatever she takes. I am worrying she might get gastrointestinal (gi) bleeding, sometimes. She is taking pantocid 10 mg (a.c.) half and an hour before every meal. Her lung is filling with fluid (water) and she gets pulmonary edema and that make her feeling shortness breath and tightness is chest and get chest pain as well. She is taking lasix (furosemide) 10 mg (q.d.) to get rid of that extra fluid retention. She gets hypertension (high blood pressure) and is taking amlodipine (norvasc) 5 mg b.i.d. And for the heart related issues, she is taking metaguard 20 mg, monotrate 20 mg (1 tablet each) a.c. When she is taking the lasix, we have to put on hold for the (amlodipine). When the cardiologist took her the e.c.g and angiography (echocardiogram), he found the narrowing of the pulmonary valve opening i.e. Pulmonary stenosis. Firstly, he suggested us to do the pulmonary valve replacement. Due to her weakness and older age, the doctor is recommended to do so. Again, the doctor mentions she might get the chest pain due to her vitamin-d deficiency. It causes the inflammation of the cartilage in the rib cage (costochondritis). She has to take pain killer to release and comfort herself. One or two months ago, she was admitted to the hospital due to her continuous chest pain and shortness and tightness of breath. Then she got fainted. May be she got mild heart attack. After the blood test and angiogram results, it stated clotting of blood in the heart. The blood clot completely obstructs a coronary artery supplying blood to her heart muscle. The doctor also stated magnesium deficiency is heart arrhythmia that was weakening her heart muscle. When her heart can't pump enough blood to meet her body's needs, blood flow slows which can cause clots to form. The doctor injected her blood thinning medication and kept her relaxed in the hospital for about two weeks with a nasal cannula attached to the oxygen supply. After that episode, her physician prescribes her clopilet a 75 mg (q.d.) for thinning blood to avoid the clotting of blood. Due to the medications, her liver gets much toxins, that make her histamine level is high can cause pruritus and dryness. She is always complaining the itchiness most of the time. Whenever she causes the episodes of angina in order to prevent the angina from occurring, we apply nitroglycerin spray to her several times either day or night. Is there any other option to release her chest pain? How about the oxygen therapy? What we should do for her great health. No one wants to see his parent severe suffering with great sorrow. Please assist me with your precious, valuable sound suggestion with earliest response. Thank you very much in advance.

I feel immensly sorry for all that she is suffering from. All that is you narrated is ageing issues. Multiple complaints one related to other mostly medication related. Vit d deficiency because she is not exposed to sufficient natural light. Electrolyte deficiencies due to certain medication. The cardiologist is doing there best, just follow his advise and be ready to accept the fact which are inevitable.
