The possible cause should ideally be the infection of TB for which he had been put on AKT4. Consult your testing physician for a fine tuning of the dosage and see if makes sense to prolong the intensive phase of AKT4 for some time before switching to scaled down version called AKT3.

The red-coloured capsule must be taken either 1 hour before breakfast or 2 hours after breakfast. After taking the capsule patients must not consume anything (except water) for at least an hour. The remaining 3 tablets can be taken with food. These tablets can be taken together, i.e., all three with breakfast or lunch or dinner. The 3 tablets can also be taken separately after different meals, but the timings should be maintained.

There is no serious aide effect of taking AKT4 longer. AKT4 when compared to AKT4 has 1 extra MEDICINE. It is called pyrazinamide. This medicine can cause hepatitis in some cases and not all (liver injury but it is temporary and can be treated). Another minor problem is gout related joint pain. None of them are serious and can easily be treated.

This may be normal as you do not have fever or loss of appetite. However your concern is justified as lymph nodes have not reduced in size. You can get a very specific answer by getting an FNAC and sending it for TB culture and a rapid test called GeneXpert/TBNAAT. This test is available in government hospitals free of cost.

Akt-4 is fixed drug combination, it may be underdose or overdose.
