What is insulin therapy?

Usually when kidney function is reduced and eGFR is <45 ml/minute, metformin may be discontinued. However, discontinuation does lead to blood glucose levels going haywire and may need increased Insulin dosage.
So pleas let me know following: serum creatinine level, height, weight to calculate your eGFR. Or if already it is mentioned please do give that info. Because on discontinuing metformin glucose level tends go up. Thanks.

Dear most important is to control your blood sugar levels. Insulin is medicine and not recreational drug that you get addicted to it.

Let us also know whether you are on any other medications provide a complete list (some medicines have a tendency to increase sugar level). hello arif .. walk is very important for diabetic and obese patient . so she have do to do daily brisk walk . have small and frequent meals. don't skip your breakfast ever. take always high protein diet and high fiber diet. take plenty of water. take one fruit daily . take salad atleast 2 times in a day. take luke warm water whole day they will help to reduce your weight. take green coffee beans water they will also help to reduce your weight.

Normally starting dose of insulin for type 2 diabetes is roughly 0.4-0.6 units/kg weight. But in type 2 diabetes insulin resistance is variable. In long standing diabetes pt whose beta cells are exhausted and who is obese insulin requirement may go beyond 2-3 units/kg. HbA1c is approx average of blood sugar control in last 3 months. More HbA1c means more uncontrolled diabetes and may require more insulin.

In t2dm there are no antibodies produced against insulin, it is the insulin resistance that is the culprit along with improper production of insulin by beta cells of pancreas. You might a drug like metformin 500 mg twice a day in addition to insulin which would reduce insulin resistance and help insulin action. Plus your strict diet and exercising for more than 1 h a day should continue.

Hello Mr. lybrate-user, Apart from taking medicines do proper exercises for your knees. Exercises like strenght training of your quadriceps especially VMO, stretching for hamstring, calf,TFl, proprioceptive training, apply ice 3 times a day if it is swelled too much, you can also apply hinged knee brace if you feel pain during walking in your knees. Consult a good qualified Physiotherapist for proper care further.

The very fact that you have been advised to take Insulin injections for controlling blood glucose, suggests that only oral drugs seem to be not working effectively. This happens in persons whose blood glucose is exceptionally high to begin with. In some cases it is possible to do away with Insulin after some 3 to 4 months, once a strict control is achieved. Since there is no mention about your current glucose levels and HbA1c%, it is difficult to comment. ...more
