Lantus insulin is long acting insulin. If patients fasting/PP Sugars are not under control and and Hba1c is persistently above 8 % then some times it is advisable to go for premix insulin like humalog + NPH insulin for better glycemic control and maybe twice in a day injections. If you have recent post heart bypass surgery then it is very important to maintain Hba1c below 7%.

Mr. Lybrate-user, if you have t1 dm, then only treatment is insulin injection, no other option. If you have t2 dm and still you have been given insulin that means your diabetes must have been really in bad control. Diabetes occurs either due to lack of insulin or resistance to its action (t2 dm), or due to total absence (t1dm). Best medicine for diabetes in fact is insulin, it has no real side effects, our own islets of langerhans in pancreas produce it.

It's very essential to have a blood sugar level that can be controlled by oral medicines. If you're taking insulin it means your sugar is not under control. Get hba1c levels checked and also fasting and post prandial blood sugars at regular intervals. If the range is coming at an acceptable level you can be put on oral medicines.

Mixtard 30/70 is 12 hr insulin and should be given twice daily.
Considering your current reports your post meal sugars are high throughout day.
Ideally you should be on basal bolus regimen (glargine as basal and rapid acting insulins like lispro as bolus before each meals)
alternatively some OHAs like voglibose or gliptins can be tried for post meal peaks. It is advisable to consult a good diabetologist for managing your sugars as more detail history needed for optimum control.
